From the outset, you will have a care and support plan in place that describes what staff need to do to ensure you always receive person-centred care.
Service users, their family and friends (where appropriate), are encouraged to be actively involved in developing the Care Plan during the Assessment, which takes place prior to care/support package being put in place. This Care Plan reflects your personal choices, wishes, feelings, likes, dislikes, hobbies and interests as well as your goals, abilities and how you would like staff to manage and support your health requirements.
Care Plans will also have contact details of whom you wish to be contacted should the need arise.
Alongside your Care Plan, you may also have a Health Action Plan in place.
When you join Rest Assured, you become our priority. We will endeavour to communicate with you regularly, on top of your care provisions, via phone, email and newsletter.
In the event of staff running late or are unable to attend, you will always be telephoned, and a staff member known to you will attend.
Should you need to visit the hospital or use another service, we can plan this with you or your family to ensure these visits go as smoothly as possible.
We know that cost is one of the biggest concerns about care, so we’ve made our service stress-free and of excellent value. Our costs are very easy to understand and manage, and you will always be firmly in control of your finances. Our visits are a minimum of one hour and often longer.* This provides plenty of time for quality care whilst still leaving time left to chat and build a real relationship.
If you, your family or friends have any concerns or complaints, we will always take these seriously and inform management, all complaints and concerns will be thoroughly investigated and responded to in a timely manner.
